Q: How do I know whether I can participate in these classes?
A: If you have been diagnosed with pre-diabetes or Type 2 diabetes, and are a benefits eligible State of Arizona employee, spouse and/or dependent ages 18 and over, you may participate in these classes. However, only employees with an Employee Identification Number (EIN) will be able to claim HIP points.
Q: Does insurance pay for these classes?
A: No, these classes are self-pay only. You will be asked for a credit card number when you register.
Q: What are “online classes”?
A: Online classes are classes you can attend via the internet. As long as you have an internet connection, you can view the course videos and materials.
Q: What are the technology requirements for participating in these classes?
A: You will need a computer, a tablet, or a smart phone. You will be able to access the class via an internet browser on any of these devices. You will need an active internet connection or cellular connection if you are using a smartphone (data usage will apply).
Q: I don’t have a computer at home; how can I participate in these classes?
A: You may participate using a smartphone or visit your local library to utilize their computers.
Q: How long do I have to complete these classes?
A: From the time of registration, you will have 3 months to complete the classes.
Q: What is the ASU Research Study?
A: You will have the option to volunteer to participate in a research study (consisting of online surveys) when you register for these classes. We will be conducting the research study with Arizona State University to evaluate the effectiveness of these classes. You are not required to participate in the study. If you decide to participate, your information will be anonymous and will only be reported as a group, not as an individual.
You will receive an invitation to participate in the study after you register for the Mindful Eating for Diabetes classes. If you participate, you will complete the surveys before you start the classes and after you finish. Each survey takes approximately 20 minutes to complete.
The Mindful Eating for Diabetes classes cost $179. However, if you elect to participate in the research study, you will receive a refund of $129 after completing the second survey at the end of the classes. This will result in the program only costing you $50.
Q: What materials will I need for these classes?
A: An online (PDF) copy of Eat What You Love, Love What You Eat with Diabetes is included in your course registration at no additional charge, or you may purchase a paperback copy for $19.95 plus $6.95 for 2-day shipping. If you elect to participate in the ASU research study (see above), we will mail you a free autographed copy of the paperback book.
Q: Will I earn HIP (Health Impact Program) points for this class?
A: Yes; by completing the requirements for these classes (see below), you will earn 150 HIP points under the disease management program challenge. Points must be logged by 12/31/18. You may receive HIP points for this program one time per year.
Q: What are the completion requirements to claim HIP points with State of Arizona?
A: In order to earn health points, you will need to:

  • Enter your Employee Identification Number (EIN) during registration so the points can be validated by the State of Arizona (see below).
  • Complete all six Mindful Eating for Diabetes online self-paced classes.
  • Complete a short quiz after each class.
  • Complete a program evaluation.

Q: How do I enter my EIN?
A: You will enter your 9-digit EIN during registration as follows:

  • Non-university employees: Enter EIN in the 9-digit format by adding leading zeros.
  • ASU employees: Enter your 9-digit ASU Campus ID/Alternate ID#. This number begins with 99 and may be found on your paycheck directly under your name/employee ID.
  • UofA employees: Enter your 9-digit Alternate Employee ID#. This number may be found on UAccess under Benefits tile and Insurance Summary link; use the Alternate Employee ID on the right. (Do NOT use the Empl ID at the top.)
  • NAU employees: Enter 77 followed by your 7-digit NAU ID number with no leading zeros. Your NAU ID number is found on Jacks card or on LOUIE/Peoplesoft account.

Q: I have completed the course but I have not received my points; what do I need to do?
A: Please email wellness@azdoa.gov.
Q: What is the registration period?
A: These classes will be offered on an on-going basis. However, you will need to complete the course before the end of the year in order to claim your HIP points.
